    This book made me lose some sleep - I could not stop reading it! I was absolutely shocked by the stories of the many people who have been wronged by our health care system in Canada! I am ashamed that these errors and omissions have happened and that there are those - my fellow Canadians - who will not admit their mistakes! At present I am seeking answers to mistakes I believe contributed to my husband's death three years ago. This book has helped provide me with some insight as to the steps others have followed in the pursuit of change to practices and procedures that may save other lives. As I read the book I saw so many of the obstacles I have faced...from the refusal to admit the errors ... right to the refusal to respond in writing to specific questions. I have the e-book on my Kindle - I plan to order a paper copy to loan out to friends....a hard copy will be easier for me to use for reference. I highly recommend this book to anyone who wishes to become better informed, to anyone who has lost a loved one and has concerns about care which may have lead to death, or to anyone presently anticipating medical procedures. You will understand that you are entitled to answers if things do go wrong - and that you may be able to help make changes which could save other lives! We have a responsibility to speak up and this book may just give you the confidence you need to ensure that your voice is heard. The authors did an excellent job in research and presenting each case so vividly. You may shed tears as you read this book .. I did!

    Stories about medical misadventures hold our attention. Research published in Canada, the United States and Australia reports a disturbing frequency of bad outcomes. Every year, more than 10% of patients experience "preventable adverse events in hospital". Thousands of patients die while trusting healthcare delivery or taking medications - as prescribed! Co-authors S. McIver, PhD (former professor and BC coroner) and R. Wyndham (a registered nurse for 34 years) introduce 16 Canadian patients and families who explored the dark side of medicine. A surprising variety of mishaps turned their hoped-for recoveries into medical misadventures. These detailed accounts warn us that harm and damage can occur if seemingly sincere health professionals make mistakes or offer substandard care.

    Aren't sick people preoccupied with aches and pains, symptoms, diseases and illness-related difficulties? Who notices if clinicians shortcut care, make careless mistakes, misdiagnose or mistreat? The 16 patients and families we meet in this book eventually realized that they or their relatives unexpectedly got worse or died after getting medical care. The survivors became medical detectives. They investigated by studying medical charts, finding clues, interviewing witnesses and reading reference books. They asked questions, obtained copies of patients' records, sought second opinions, organized evidence and documented problems, errors and omissions. They noticed misdiagnoses, mistreatments, mistakes and other mishaps involving miscommunication, problems with medication, incompetence, neglect or negligence involving health professionals.

    McIver and Wyndham write, "These people had experienced harm to themselves or to their loved ones, often with fatal outcomes. These people wanted to direct their grief, anger and sense of injustice in a way that would result in safer care for every patient." After recovering or burying their dead relatives, these patients and families decided to educate the public. However, they soon learned that health professionals and hospitals are shielded by risk managers and medical protective associations which typically maintain a polite silence after victims report medical errors, damage or death. These 16 mishap survivors and their families faced roadblocks, stonewalls, silences, denials, deceits and other difficulties but they kept telling their stories to physicians, nurses, administrators, regulatory authorities and the media - while coping with loss, pain and suffering.

    These courageous patients and families reported substandard care hoping that their stories would inspire positive progress, help to improve systems and procedures and protect other families. Their stories teach us that survivors of medical mishaps can advocate for patient safety and share their experiences by (1) writing books, articles and websites, (2) speaking at meetings, workshops and conferences, (3) appearing on radio, TV and film, (4) networking in-person and on-line and (5) volunteering at not-for-profit organizations. Even if it takes years, the truth can come out; lessons can be learned and steps taken to protect other vulnerable patients.

    These medical misadventure stories will make you cry, but the remarkable perseverance, resilience and resourcefulness of the 16 survivors will encourage readers to become educated healthcare consumers. Anyone can read about medical conditions and learn standard procedures for diagnosis and treatments. Guided by this book, patients and families can spot mishaps-in-progress and alert health professionals and hospitals in time to solve problems, improve care, minimize suffering and save lives!
